Understanding a discharge from a hospital or rehab center can be very confusing and overwhelming. You may have to set up new services or become familiar with new equipment your loved one needs to return home. This can be confusing, intimidating and often is the reason these loved ones stay at nursing homes long term. If you need help clarifying information, learning what equipment would be helpful, what you should have set up ready to go at home or even just words of encouragement then this service is for you!

Often times after a surgery, fall, or major incident a patient may need new or additional equipment to live independently. Most insurances only cover one piece of equipment upon discharge even though a patient may need more than one to transfer home safely. Costs can add up quickly and lack of equipment should not be the reason you or your loved one cannot return home. We can provide you with new or donated equipment as funds allow. In addition, we can also provide training and education about the device and how to properly use it.

Being a caregiver is a very difficult job. It is physically, emotionally and mentally draining. Most people that become caregivers for their loved ones do so in addition to balancing their full time jobs, taking care of their own family and countless other priorities. This can become very draining and lead to caregiver burn out and ultimately lead to loved ones being admitted into a long term care facility. At this time we are working to create a caregiver support group over on our forum page. Click the forum button below to contribute to an existing discussion or start a new one. We hope to expand our support for caregivers in the future like respite care (see more details below).

Financial Support

The most common reason someone will be admitted into a long term care facility is because they are unable to obtain or pay for the necessary amount of caregiver services at home. Full time caregiver services are expensive and often times insurance will not cover the full cost or only for a short period of time. We can discuss the option of providing financial support towards hiring the necessary care services at home. Make an appointment with us to discuss in more detail!

Future Services

Below are future services we are eager to add to our programs to further improve the quality and possibility of care at home for older adults . These services are not available at this time due to support and funding limitations. Donations helps us grow and the more we grow, the more people we can help.

In-home Therapy Services

We may be a little biased because our founder is a physical therapist but, therapy is very important! While insurance does cover in-home therapy, often the service is prematurely discontinued. We provide in- home PT, OT and SLP services for those who would continue to benefit from more therapy. ​

Home Modification

Often times older adults cannot go back home because their home setup is not safe or accessible for their new level of function. Dependent on availability and funds, this service would provide financial assistance or free labor and materials to perform those home modifications. This could be something as simple as installing hand rails to a full bathroom remodel.​

Respite Care

Being a caregiver for a loved one is a rewarding but draining job. Caregiver burn out is a main reason older adults move into long term care facilities. By providing respite care services we hope to prevent burn out by arranging breaks for caregivers. This could be for a simple date night or for several weeks as you travel around Europe.
